抗體是存在于體液和淋巴細胞表面上的能與抗原特異性結(jié)合的免疫球蛋白??贵w是由B細胞分化成的漿細胞產(chǎn)生的??贵w分子具有結(jié)合部位(結(jié)合簇),能與相對應(yīng)的抗原決定簇結(jié)合。由于不同的抗原與抗體特異性結(jié)合表現(xiàn)出各種不同的反應(yīng),因而抗體有各種不同的名稱,如凝集素、沉淀素、抗毒素、溶血素、溶菌素、補體結(jié)合抗體等。這些結(jié)合,在適當(dāng)?shù)臈l件下,可發(fā)生直接或間接的可見反應(yīng),可作為臨床診斷的一種方法。抗體有作用于相對應(yīng)的細胞、組織、毒素和酶等一系列的生物學(xué)活性,與對應(yīng)抗原在機體內(nèi)結(jié)合后,可以被吞噬、排泄而將抗原清除,或使抗原失去致病作用,故常用以防治某些疾病。這在醫(yī)學(xué)應(yīng)用中,曾發(fā)揮了巨大的作用。在另一些情況下,抗體與抗原結(jié)合形成的免疫復(fù)合物,能損傷組織、細胞,引起超敏反應(yīng)或免疫性疾病等不良后果??贵w過去統(tǒng)稱為丙種(γ)球蛋白(γG,GG),這是因早年用電泳法分析血清時看到抗體活性在γ部分而命名的。后來證明,抗體并不都在γ區(qū); 反之,位于γ區(qū)的球蛋白也不一定都具有抗體活性。1964年世界衛(wèi)生組織專門會議將具有抗體活性以及與抗體相關(guān)的球蛋白統(tǒng)稱為免疫球蛋白(Ig)。目前多數(shù)學(xué)者認為抗體這一名詞只表明它的生物學(xué)活性,如在靶抗原已被確定的場合??膳c靶抗原特異性結(jié)合,但不能表明它的化學(xué)本質(zhì)。而免疫球蛋白一詞既意味著有免疫活性,更著重表示它的化學(xué)含義,是指單一的分子。不嚴格區(qū)分時,二詞可以混用。B淋巴細胞受抗原刺激后,怎樣變成能夠分泌抗體的漿細胞或轉(zhuǎn)變成記憶細胞,這些機制還遠未搞清。


抗體的產(chǎn)生有一定的規(guī)律。當(dāng)抗原第一次進入機體時經(jīng)過一段潛伏期,抗體效價逐步上升,達到高峰后,維持一短暫的時期,以后又逐漸下降,此為初次應(yīng)答或初次抗體應(yīng)答。但在第二次再接觸同樣抗原后,開始時表現(xiàn)為原有抗體量的略為降低。這是因為原有抗體的一部分與再次注入的抗原結(jié)合的緣故。隨后抗體效價的上升較初次為快,其高峰時的效價也比初次為高,持續(xù)的時間也長,此即所謂二次(抗體)應(yīng)答或再次(抗體) 應(yīng)答。由抗原刺激所產(chǎn)生的抗體,經(jīng)過一段時間后即逐漸消失,以后再接觸抗原,可使已消失的抗體迅速上升,是為回憶應(yīng)答。廣義的回憶應(yīng)答可包括二次應(yīng)答。若再次刺激的抗原與初次相同,則為特異性回憶應(yīng)答,若與初次不同,則為非特異性回憶應(yīng)答。非特異性回憶應(yīng)答的抗體只是暫時性上升,短時間內(nèi)即很快下降。
抗體的一般特性包括: ①特異性: 抗體只能與相應(yīng)抗原發(fā)生特異性結(jié)合,而不能與其他無關(guān)的抗原結(jié)合。這是由抗體的結(jié)合部位與抗原決定簇的理化性質(zhì)恰相對應(yīng)所決定的。②不均一性: 抗體是一組不同種類的免疫球蛋白分子所組成(參見“免疫球蛋白”)。它們的氨基酸組成和排列以及立體構(gòu)型既相似又略有差別,因而其大小、形狀和功能也互有差異。例如天然抗原性物質(zhì)的表面含有許多不同的決定簇,由其刺激機體產(chǎn)生的抗體,就包含著針對這些不同決定簇的抗體。例如抗傷寒桿菌的抗體,至少應(yīng)包括抗菌體的O抗體,抗鞭毛的H抗體以及抗毒力 (Vi)抗原的Vi抗體; 事實上針對菌體的抗體,由于菌體抗原(9,12)不同,其相應(yīng)抗體也互不相同。就是針對某一決定簇的抗體,也包括IgG、IgA、IgM、IgD和IgE五大類及其亞類,雖然在某一個體內(nèi)不一定同時都有。對某一類抗體(如IgG)來說,不同種系以及不同個體動物間所產(chǎn)生的抗體也都不完全相同。這種不同是由遺傳決定的??贵w的這些不均一性,在臨床上都是必須注意的問題。③雙重性: 抗體一方面可與抗原特異性結(jié)合,發(fā)揮其免疫功能; 另一方面,由于它本身是高分子蛋白質(zhì),當(dāng)被注入異種動物體內(nèi)時,就具有抗體和抗原的雙重作用。在用異種動物的抗血清反復(fù)注射以防治疾病時,必須注意有發(fā)生過敏反應(yīng)的可能性。關(guān)于抗體產(chǎn)生的機制,曾有過許多假設(shè)和學(xué)說。


人類免疫球蛋白已鑒定出5種,它們是:IgG,IgA,IgM,IgD和IgE,差異發(fā)在H鏈上,分別由稱為γ、α、μ、δ和ε的H鏈構(gòu)成。
IgG是人類血清中的主要免疫球蛋白??乖ぐl(fā)后產(chǎn)生的主要抗體類型,就是這種單體。胎兒雖不產(chǎn)生IgG,但它能迅速通過胎盤,故胎兒和新生兒血清中皆能檢出母體的IgG。根據(jù)重鏈的不同,IgG可分為4個亞類:γ1,γ2,γ3,γ4。有些抗原激起的抗體反應(yīng),4亞類可說旗鼓相當(dāng),但有些抗原則是以其中某一亞類為主。作用于糖類的抗體常為IgG2;抗DNA抗體主要是IgG1和IgG3。在各種亞類中,IgG3的合成速率最低,而分解速率最高,血清半衰期最短。有關(guān)IgG的各種生物學(xué)和物理特性,均見表1及表2。
IgA有單體也有二聚體。它是分泌物中的主要免疫球蛋白,并以二聚體存在,后者是以多肽連接鏈(J鏈)和二硫橋互連的。此外,分泌物(涎液、淚液、腸液中的二聚體,還含有1條稱為分泌成分(SC)或分泌片(secretary piece)的多肽鏈。SC似能保護二聚體IgA,免于胃腸道蛋白水解酶的消化作用。它是由局部漿液型分泌性上皮細胞產(chǎn)生的,位于這些細胞的表膜,這使人想到:SC可能是二聚體IgA的受體。SC片越過或由這些細胞間通過時,不斷增加,據(jù)認為,分子之能藉胞泌作用(與胞飲作用相對)通過粘膜上皮,即緣于此。因此,分泌性IgA的生成,就可認為是不尋常的,因為它是兩種不同類型細胞(即抗體分泌性漿細胞和漿膜上皮細胞)的協(xié)作產(chǎn)物。按其α重鏈的不同,IgA可分為IgA1和IgA2兩亞類。后者又可按其遺傳標記(genetic markers)的不同,分為A2m(1)和A2m(2)兩亞種。
IgM是最大的免疫球蛋白(分子量890,000),為5個相同亞單位組成的五聚體,各有兩條K或λ輕鏈和兩條μ重鏈。此外,IgM還有1個與二聚體IgA相似的J鏈,使分子結(jié)合更為牢靠。IgM對補體結(jié)合至為有效,對紅細胞和細菌的凝集作用以及殺菌反應(yīng),也比IgG有效得多。單體IgM見于B細胞表面,據(jù)信是起抗原受體的作用。初次給予1劑抗原后,經(jīng)過一段起始延擱,首先出現(xiàn)的抗體就是IgM,一般于7日到達頂峰。但此時IgG抗體亦可檢出,一般于10~14日到達最高效價。在IgG效價增長期間,IgM效價開始下降,迨至初次抗原劑量4~5周后,已難以檢出。2次反應(yīng)(回憶反應(yīng)或增強反應(yīng))時,IgM和IgG效價皆呈指數(shù)性增長,但IgG反應(yīng)顯然更強,持續(xù)也更久;IgM效價則與1次反應(yīng)時相同,或略有增高。產(chǎn)IgM細胞記憶功能不佳,雖經(jīng)2次抗原激發(fā),亦無典型回憶反應(yīng)。故IgM的存在有助于確定是否為新發(fā)感染。有證據(jù)提示帶IgG或IgA淋巴細胞是由帶IgM或IgM/IgD淋巴細胞前體發(fā)育來的。
IgD是由兩條δ重鏈和兩條κ或λ輕鏈構(gòu)成的單體。在血清免疫球蛋白中,含量甚微,合成速率大約不到IgD的1/100。IgD的分解速率亦高,血漿半衰期只有3日。IgD不與補體結(jié)合,也不對皮膚致敏。大多數(shù)臍帶和成人的B細胞表面,都有IgD,有人認為IgD可能是一種早期抗原細胞受體。
IgE是由兩條ε重鏈和兩條κ或λ輕鏈構(gòu)成的大單體。血清含量至微,血漿半衰期最短,合成速率最低,分解速率在各種免疫球蛋白中也是最快的。IgE在即發(fā)型超敏反應(yīng)中起主要作用。它以Fc部分緊密地結(jié)合在肥大細胞和堿性細胞的受體上。繼而抗原(變應(yīng)原)與臨近二IgE分子相互作用,形成抗原橋。由此引起的局部膜改變,促使介質(zhì)釋出(組胺、慢反應(yīng)物質(zhì)、酸性細胞趨化因子),從而觸發(fā)即發(fā)型超敏反應(yīng)。特應(yīng)性(atopic)者的變應(yīng)原特異性IgE水平增高。據(jù)認為IgE對寄生蟲有保護作用,但尚無確切資料得以證實。
